Welcome to Northern Oklahoma College. We are excited that you have decided to attend NOC and to compete in NJCAA intercollegiate athletics. As a student-athlete at NOC and in the NJCAA you are required to provide certain documentation in order to be eligible to compete. A portion of this includes current address, emergency contact, insurance, medical alert and health history information, physical, eligibility affidavit, and various consent forms. To expedite this process Northern Oklahoma College uses an online data entry system.

Please read and fill out each form very carefully. The accuracy of the information that you provide is very important. Be sure that each document includes all of the required dated signatures.
If you have a question about any of the information that is being requested, please contact your coach at NOC or the Director of Athletics.
Several sections require an electronic signature. Simply put the date you are completing the form in the date section and your name if you are 18 years of age or older. If you are under 18 years of age your parent/guardian will need to complete the electronic signature. Again, be sure that each applicable document has the required dated signatures. Be sure to select save and submit for each form. These documents should be saved and printed for your records.
Students will not be permitted to participate in any physical activity until documentation of a physical exam has been submitted. Northern Oklahoma College does not provide physical exams or funds for physical exams to student-athletes.
This letter is only designed to assist you in completing all necessary items for athletic participation. Keep in mind that there are also a number of equally important items that require interaction with other campus offices.
