Important Forms for Veterans (Active Duty, National Guard, and Reserve):

 

Note: You can also fill out these GI Bill Applications ONLINE.

 

Applying for the first time for Benefits:

First time application for Veterans/National Guard Members/Reservists for Chapters 30, 33, or 1606 (VA Form 22-1990)

This is the form you need to use to apply for benefits under the GI Bill for the very first time. You will need to fill this form out and send it to Brad Gordon at the Tonkawa campus. We will then send it to the VA for you.

 

Changing Schools:
Request for Change of Program or Place of Training (VA Form 22-1995)

This is the form you use if you are CHANGING MAJORS or if you have previously received GI Bill benefits at another institution. It is essential to use this form when you are changing majors because the VA requires us to only certify you in classes that will count towards your major.

 

Transferring Post-9/11 (Chapter 33) benefits to a dependent or spouse:

Application for Family Member to use Transferred Chapter 33 Benefits (VA Form 22-1990e)

This is the form that transferee must fill out to receive transferred (Chapter 33) Post-9/11 GI Bill benefits from their parent or spouse.  

The transferor (Veteran) must first visit the following website and request to transfer their benefits to their dependent(s) or spouse: Link to DOD Transfer of Eligibility Website

Veterans Advisory Form (Must be completed EVERY semester):

Veterans Advisory Form-ONLINE

This is the form you have to fill out EVERY SEMESTER. Please fill it out completely and send it to Brad Gordon in Tonkawa, or click the link above to submit your form electronically. Without this form from you, we assume that you are not interested in receiving benefits from the VA and we will not certify your classes with them. This has a direct effect on your VA check!

NOC Change of Program Form: If you are a current NOC student and are changing your major, please fill out this Change of Program Form:

Important Forms for Dependents and Spouses of Veterans:

Applying for Dependents Educational Assistance (Chapter 35) for the first time:

First time application for Dependents’ Educational Assistance (Chapter 35 or Chapter 33 Fry Scholarship Recipients (VA Form 22-5490)

This is the form you use to apply for GI Bill benefits if you have never drawn benefits at another institution.  Please print this form off and fill it out completely in pen, and don’t forget to sign it.  Send the form to Brad Gordon at the Tonkawa campus and we will send it to the VA for you.You can also fill out the GI Bill Application ONLINE .  If you apply online, please send a copy of your application to us, so we will have it in your file.

This is the form you use if you are CHANGING MAJORS or if you have received benefits at another institution and are transferring your benefits to NOC.  It is essential that you use this form if you change majors because we can only certify classes that are required for your major.  If you have not changed your major with the VA, you may be taking classes that will not be certified.
