Student Complaint Form for Official Grievances
An official grievance is one in which students submit a written, signed complaint through the form below to the Office of Student Affairs, where it will be directed to the appropriate supervisor. Official grievances are distinguished from all of the following:
- Suggestion Form
- Appeals that can be resolved through departmental processes
Academic Appeal - Students who wish to challenge an assignment or course grade should follow the procedure described on the NOC website at the following link: Academic Policies & Procedures
Financial Aid Appeal - Students who wish to appeal loss of financial aid eligibility should follow the procedure described on the NOC website at the following link: Satisfactory Academic Progress
Scholarship Appeal - Students who wish to appeal loss of scholarship eligibility should complete the appeal form found on the NOC website at the following link: Scholarships
Tuition/Fees Appeal - Students who wish to request a refund or balance waiver of their tuition and/or fee charges due to extenuating circumstances preventing attendance should follow the procedure described on the NOC website at the following link: Tuition & Fees
Complaint against an NOC employee - Students who wish to lodge a complaint against an NOC employee should first attempt to resolve the issue with the employee directly; if the issue is not resolved, the student should contact the employee’s supervisor if known.
SARA Policy Complaint - NOC participates the State Authorization Reciprocity Agreement (SARA), and any student attending NOC by exclusively distance education while living outside Oklahoma (except those in California) are attending NOC through SARA (SARA student). An NOC SARA student who believes that NOC is operating out-of-compliance with SARA policy should pursue the complaint through NOC's regular student complaint/grievance process. If, after that process is complete, the student believes that NOC's resolution of their complaint is not appropriate or NOC is still operating out-of-compliance with SARA policy, the student can appeal NOC'S resolution with the Oklahoma State Regents for Higher Education (OSRHE) through its OSRHE SARA Student Complaint procedure. The Official Grievance Form should be used for issues not addressed above or ones not resolved through the processes above. Because of their serious nature, any complaint involving ADA compliance or a violation of Title IX Guidelines related to sexual harassment should be documented as an official grievance.
