Student Complaint Form for Official Grievances

An official grievance is one in which students submit a written, signed complaint through the form below to the Office of Student Affairs, where it will be directed to the appropriate supervisor. Official grievances are distinguished from all of the following:

  • Anonymous Suggestion – These can be submitted on the NOC website at the following link: www.noc.edu/suggestions
  • Appeals that can be resolved through departmental processes

Academic Appeal – Students who wish to challenge an assignment or course grade should follow the procedure described on the NOC website at the following link: www.noc.edu/academic-policies–procedures Financial Aid Appeal – Students who wish to appeal loss of financial aid eligibility should follow the procedure described on the NOC website at the following link: www.noc.edu/satisfactory-academic-progress3 

Scholarship Appeal – Students who wish to appeal loss of scholarship eligibility should complete the appeal form found on the NOC website at the following link: www.noc.edu/scholarships Tuition/Fees Appeal – Students who wish to request a refund or balance waiver of their tuition and/or fee charges due to extenuating circumstances preventing attendance should follow the procedure described on the NOC website at the following link: www.noc.edu/bursar Complaint against an NOC employee – Students who wish to lodge a complaint against an NOC employee should first attempt to resolve the issue with the employee directly; if the issue is not resolved, the student should contact the employee’s supervisor if known.

The Official Grievance Form should be used for issues not addressed above or ones not resolved through the processes above. Because of their serious nature, any complaint involving ADA compliance or a violation of Title IX Guidelines related to sexual harassment should be documented as an official grievance.

  • Description of Incident or Concern

    Please provide a detailed description of the incident/concern using specific concise, objective language (who, what, where, when, why, and how). Please also include any history of the incident. E.g. What steps have been taken previously to remedy this situation? Who has been contacted and when? Full name of individual(s) involved (student, faculty or staff), etc.
